Outpatient Rehabilitation Services
The Outpatient Rehabilitation Services include a variety of clinics and interdisciplinary therapy services specific to your rehabilitation needs. Services are provided for children, adolescents and adults. Typical diagnosis treated in the program include brain injury, stroke, spinal cord injury, major multiple trauma, orthopedic disorders, Guillain-Barre Syndrome, neuromuscular diseases, multiple sclerosis, polyneuropathy, limb amputations, arthritis, debility, medically complex, burn injuries, spina bifida, cerebral palsy, and post-polio.
Outpatient Physical Medicine and Rehabilitation Clinic Schedule and Information
The Outpatient Physical Medicine and Rehabilitation Clinic is located in Valley Specialty Center at 751 South Bascom Avenue, Suite 110, San Jose, California. This clinic provides care specific to your rehabilitation needs and includes equipment evaluation, education and ongoing rehabilitative medical care.
A board-certified rehabilitation physician (physiatrist) and rehabilitation nurse will spend time with you to review your present status, make recommendations and help you with any problems you want to discuss. You may receive prescriptions for medication, supplies, equipment, and equipment repairs. You may be referred for psychological, social, or sexual counseling as well as home nursing services, therapies, or vocational rehabilitation counseling.
Usually the clinic appointment takes between one and two hours. However, if x-rays, lab tests or any special procedures are necessary, or if a particularly difficult problem exists, it could take longer.

Outpatient Therapy Services
Our therapists provide a multidisciplinary team approach for adults and children who have experienced stroke, brain injury, spinal cord injury, Guillain-Barre syndrome, amputation, muscular dystrophy, ALS, Parkinson’s disease, balance and vestibular disorders, as well as general rehabilitation diagnoses.
You may be referred by a physician directly after an inpatient hospital stay or from your primary care provider. This referral may include one or multiple services offered by our rehabilitation team. Our therapists will develop a customized treatment plan to help you achieve optimum functional ability, mobility, and independence.
Physical therapists can provide evaluation and treatments to improve strength, flexibility, coordination, balance and mobility skills. Treatment focuses on gait training with use of assistive equipment/devices as necessary, therapeutic exercises, balance training, and functional mobility training.
Occupational therapists can provide evaluation and treatments to restore independence with everyday living activities such as dressing, bathing, arm and hand function, visual perceptual skills and other self-care activities. Our occupational therapists, along with durable medical equipment vendors, also address complex seating and positioning needs and wheelchairs in our wheelchair clinic.
Speech-Language pathologists can provide evaluation and treatments to improve communication, swallowing, and cognition. Treatment focuses on speech and language retraining, cognitive and memory retraining, swallow/dysphagia evaluation and treatment, and instrumental swallow assessments such as fiberoptic endoscopic evaluation of swallowing (FEES) and video fluoroscopy.
