Our Programs

This program has been established through partnerships with Santa Clara County Parks, San Jose City Parks, Veggielution, Garden to Table Silicon Valley, and the Guadalupe Park Conservancy.

Los Capitanes Del Bosque
The Capitanes Del Bosque (Captains of the Forest) program encourages JUNTOS youth participants between the ages of 9-12 years old to explore nature and discover Santa Clara County and City Parks’ special places while fostering a relationship with the environment and Park staff. Participants observe, interact with, and learn about the unique natural and historic resources of Santa Clara County Parks. The program encourages stewardship and leadership of our natural, cultural, and historic resources.
The Capitanes Del Bosque program was developed in collaboration with our partners at Santa Clara County Parks who facilitate the program along with San Jose City Parks and the Santa Clara County Public Health Department.
The Promotores De Salud Program
The Promotores de Salud Program will train parents who have participated in the JUNTOS program to serve as community health workers who can provide guided hikes, gardening information, and other nature-related activities to the community. The goal of the program is for the Promotores to connect community members to resources and become a trusted source of information about accessing parks and green spaces in the community. Participants will be paid for their time training and working in the Promotores program.
The Promotores de Salud Program has been developed in collaboration with our partners at the Veggielution Community Farm who facilitate the program along with the Santa Clara County Public Health Department.

The Jump Park Prescription Program
The JUMP program is led by one of our PHLC nutritionists along with members of the Santa Clara County Public Health Department’s CalFresh Team.
JUMP team members meet families weekly in one of four neighborhood parks located near one of our SCVMC Clinic locations.
Each session includes:
- Nutrition education
- Peer support
- Evidence-based health education activities including CATCH games.
ParkPOWER Program for Primary Care
To promote access to safe green spaces for our entire pediatric patient population and their families, we’ve developed our ParkPOWER program for Primary Care Pediatricians. Through ParkPOWER all of our primary care Pediatricians can prescribe parks to their patients. The prescription includes access to a list of suggested guided and self-guided outings that are updated each quarter, and an annual parking pass for free access to all Santa Clara County parks.
This program has been made possible through the generous support of Santa Clara County Parks with suggested hikes coordinated by the Department of Public Health.
