Santa Clara Valley Medical Center’s Trauma to Triumph Program works with the City of San Jose Parks, Recreation and Neighborhood Services to provide violence intervention services to patients who have received medical treatment for their injuries at the hospital.
To be eligible for the Trauma to Triumph Program, patients need to be 13 to 30 years of age and have sustained an injury from one or more of the following circumstances:
- Physical Assaults
- Gunshot Wounds
- Stab Wounds
- Assault with a Weapon
Even though being a resident of Santa Clara County is essential, the program has relationships with neighboring counties and referrals may be accepted.
Participation in the Trauma to Triumph Program is completely voluntary.
Services Offered
City of San Jose Peer Intervention Specialists are trained to provide guidance and support to their clients. They can also provide assistance in getting services in other areas, including:
- Court Advocacy
- Criminal Record Clearance
- Education Support
- Employment Support
- Food Bank Services
- Housing Assistance
- Mentoring Services
- Mental Health Services
- Tattoo Removal
- Victim Witness Program Advocacy
The Peer Intervention Specialists recognize that every client has unique needs and work with their clients to identify community resources to meet specific needs.
Referral Process
The Trauma to Triumph Program refers patients to the City of San Jose. After receiving a referral, a Peer “Intervention Specialist” from the City is sent to visit the patient at their bedside. Based on needs identified by the patient, the Intervention Specialist will assist in coordinating services for the patient and their family. Individuals in the program will be intensively followed for up to one year.
In order for a patient to be referred to City of San Jose services, the patient must agree to authorize the disclosure of protected health information. This information may contain medical treatment and demographic information.
