About the DAISY Award?
The DAISY (Diseases Attacking the Immune system) Award is an international recognition program that honors and celebrates the skillful, compassionate care nurses provide every day. The DAISY Foundation was established by the family of J. Patrick Barnes after he died from complications of the auto-immune disease ITP in 1999. During his hospitalization, they deeply appreciated the care and compassion shown to Patrick and his entire family. When he died, they felt compelled to say “thank you” to nurses in a very public way.

How to Nominate a Nurse
Honorees are nominated by patients, families, colleagues and with use of a criteria, chosen by peers.
There are 3 methods to submit a nomination form:
- To access an electronic nomination form or a PDF (print file), go to www.scvmc.org/DAISY
- Scan the QR code located on DAISY posters in elevators, hospital entrances or DAISY buttons worn by nurses.
- While in the hospital building to go the nearest nurse’s station and ask about how to recognize and nominate your nurse for the DAISY Award.
