How to Nominate a Nurse

Honorees are nominated by patients, families, colleagues and with use of a criteria, chosen by peers.

There are 3 methods to submit a nomination form:

  1. To access an electronic nomination form or a PDF (print file), go to scvmc.scvh.org/DAISY 
  2. Scan the QR code located on DAISY posters in elevators, hospital entrances or DAISY buttons worn by nurses.
  3. While in the hospital building to go the nearest nurse’s station and ask about how to recognize and nominate your nurse for the DAISY Award.
