Health IT Developers seeking to provide access to Health Link USCDI/EHI for patient end users can access published Application Programming Interface (API) endpoints for the Santa Clara production environment at open.epic.com.

Health IT Developers who wish to obtain USCDI/EHI from CSCHS without using API standards (for example, a custom interface) must submit a written request through the Data Access Request (DAR) Process. You can download the form here. A tip sheet on completing this form can be found here.
