Internal referrals should be placed in HealthLink based upon the Orthopaedic Department Referral Guidelines​. 

Outside referrals not entered into HealthLink should be faxed to the authorization center at (408) 793-1892.

Santa Clara Valley Medical Center accepts Medi-Cal, Healthy Families and Healthy Kids health plans, and most major health insurance plans except PMG, Prime and out of county Medi-Cal.
