A PROGRAM FOR PATIENTS WITH CATASTROPHIC INJURIES

Neuro-Net provides patients expert help navigating recovery for 9 months following discharge from the SCVMC Rehabilitation Center. Participants will work with an experienced case manager who will provide real time help with:

• Coordinating follow-up medical care
• Improving communication between rehabilitation providers
• Re-entry into the community
• Advocating for your rehabilitation needs with insurance companies​

OUR GOAL FOR YOU

The goal of the program is to create a new system of integrated care to better help patients and families navigate through their recovery journey.

We will be gathering outcomes information to help us better understand the things that are most helpful in keeping patients healthy, happy, and engaged with the community. At the same time, participants in the study receive help along every step of the journey.

The journey starts with Neuro-Net.

WHAT IS NEURO-NET?

Individuals suffering a catastrophic injury or illness need specialized ongoing care to help them return to the best lives possible. Santa Clara Valley Medical Center, a leader in rehabilitative care for over 40 years, has partnered with Learning Services, Care Meridian, and Rehab Without Walls, to provide a program designed to improve help for patients and their families through the first 9 months after injury.

BENEFITS OF PARTICIPATION

Expert help navigating recovery for 9 months from an experienced case manager, including:

• Coordinating follow-up medical care
• Improving communications between rehabilitation providers
• Advocating for your rehabilitation needs with insurance companies
• Knowing that you are providing information to help create a new system of care for persons with catastrophic injuries 

HOW NEURO-NET WORKS FOR YOU

• You'll have regular contact with a case manager by phone
• If you have questions or concerns that aren’t medical emergencies, you can call your rehab case manager for advice
• Your case manager:
  • Will help coordinate all your transitions of care from one facility to another, or to home
  • Is available to visit you in person, if needed
  • Will be available to attend important medical appointments with you
  • Is an expert in many community resources and will help facilitate accessing these resources

Your Neuro-Net rehab case manager is intended to be a trusted partner in rehabilitation with you and your family, your health insurance, and the facilities and providers where you receive medical treatment.

WHO IS ELIGIBLE TO PARTICIPATE?

Men and women with a diagnosis of:

• Brain injury or spinal cord injury
• 18 years of age or older
• Have private insurance, including worker’s compensation
• Live in Northern California
