- Patient’s name, age, and working diagnosis
Adult Trauma
Physicians treating adult patients with the following injuries should transfer their patients to SCVMC:
Central Nervous System Injuries
- Penetrating Injury or open fracture
- GCS <14 especially those on blood thinning medications
- Spinal Cord or major vertebral injury
Chest Injuries
- Major chest wall injury with >3 rib fractures and/or pulmonary contusion
- Penetrating chest injury
Pelvis/Abdomen Injuries
- Pelvic ring disruption
- Concern for splenic, liver, renal, pancreatic, or other solid organ injury
Major extremity Injuries
- Fracture/dislocation with loss of distal pulses and/or ischemia
- Open long bone fractures
- Two or more long bone fractures
Any Burn/Trauma Injuries
Any major injury to two or more body regions or Multiple System Injury
Pediatric Trauma
Patients with the following injuries should transfer their patients to SCVMC:
CENTRAL NERVOUS SYSTEM INJURIES
- Penetrating Injury or open fracture
- GCS <14 with our without abnormal CT Scan
- Spinal Cord or major vertebral injury
CHEST INJURIES
- Major chest wall injury with >3 rib fractures and/or pulmonary contusion
- Penetrating chest injury
PELVIS/ABDOMEN INJURIES
- Pelvic ring disruption
- Concern for splenic, liver, renal, pancreatic, or other solid organ injury
MAJOR EXTREMITY INJURIES
- Fracture/dislocation with loss of distal pulses and/or ischemia
- Open long bone fractures
- Two or more long bone fractures
