How to Make a Referral

Thank you for considering Santa Clara Valley Medical Center for your patient’s care. Santa Clara Valley Medical Center (SCVMC) accepts referrals for both inpatient care and outpatient services for a broad range of health issues.

SCVMC is contracted with a number of health plans for outpatient services. Please take a look at our Outpatient Specialty Services and Contracted Health Plan pages for a complete list of services and plans.

Making a referral for outpatient specialty services is easy. 

Here's what you need to do:

1. Make sure your patient is a member of one our contracted health plans​.
2. Fill out the Referral Form ​or Submit EpicCare Link Access & Request Form.
3. Complete the documentation and submit the referral according to the instructions.​
