PRIMARY CARE ACCESS PROGRAM (PCAP) FORMULARY
Here is the PCAP Formulary to be used for PCAP enrollees needing medications. Medications dispensed at the Santa Clara Valley Medical Center (SCVMC) pharmacy will help to control costs and mail order services are available for enrollees’ convenience. Mail order services may be arranged at the SCVMC Pharmacy.
PCAP patients who are new to the PCAP pharmacy, will need to physically visit the pharmacy to pick up their first prescription. While in the pharmacy they may inform the pharmacy staff that they would like to utilize mail order service for their refills.
In addition, when PCAP enrollees call the telephone line 408-977-3500 for refills, they are given an option and encouraged to use pharmacy mail order services. Enrollees may also call the SCVMC Pharmacy’s Ambulatory Medication Services at 408-977-3503 to speak with a pharmacy staff member.
