Requesting Access to EpicCare Link

Sending your referral through our Electronic Health Record (EHR) EpicCare Link System can improve care coordination, communication and access to the Santa Clara Valley Health & Hospital System. We encourage you to request access to our EHR Link System and complete a Participant Access Agreement (PAA).
