Re-Entry Mobile Health Center for People Recently Released from Jail or Prison

The Re-Entry Mobile Health Center is located on a mobile bus and provides medical and mental health care services to persons recently released from prison or jail. Social workers also offer a large number of social support services, like help with income, food, shelter, and transportation. For patients who need extra help, we can pair them with a case manager, which is a person who can help with things like making appointments, giving patients a ride to their appointments, and getting medications from the pharmacy. The Re-Entry Clinic also offers psychiatry, counseling, nursing, and pharmacy services.
